How We Helped Narita and Sandy

Support coordinator Narita worked with Sandy. Sandy’s needs meant that she needed face-to-face contact in the care facility where she lived. We supported Narita by attending meetings in the care facility and met with Sandy and her case workers to help coordinate Sandy’s services.

We were able to ensure that Sandy, Narita and her case workers were comfortable with the schedule of services that would be accessed. We were also able to identify other services that Sandy could access within our community and business networks.
